Application Procedure
Complete the regular CCM Admissions Application: You may apply online at:
http://www.ccm.edu/admissions/apply
or
download
a paper application and forward it by mail.
Complete the Disabilities Services Application. You cannot complete this online. Please

to print a copy to complete.
Include all needed documentation with your Disability Services Application. For a list of applicable documentation please
click here

Mail your completed application along with needed documentation* to:

*Child Study Teams may send the supportive documentation under separate cover.
PLACEMENT TESTING
Once your disability file is complete, wait for a letter from the Disability Services Office regarding your placement test. (The letter will come in a specially marked envelope and list your approved accommodations for the test.)
Once you receive your letter, phone the Disability Services Office to schedule an appointment to take your Placement Test (with accommodations).
PLEASE NOTE: The Admission Office will also send you a letter regarding the Placement Test. Please wait until you receive the letter from the Disability Services Office. You must take your Placement Test with the Disability Services Office in order to receive accommodations.
