MANDATORY RESPONSE REQUIRED
The School Employees’ Health Benefits Program (SEHBP) has retained Aon Consulting to perform a Dependent Eligibility Verification Audit (DEVA) which will require every employee and retiree who covers a dependent under their health plan to provide specific legal documentation confirming that their dependents are eligible for coverage under the SEHBP.
County College employees should expect to receive this letter from Aon Consulting mailed to their home address sometime this fall or winter.
DO NOT IGNORE THIS REQUEST
If you do not provide the requested information to Aon Consulting within the specified time frame noted in the letter, you risk losing dependent health coverage.
They will be asking for photocopies of the following documents:
1) Marriage Certificate AND a copy of the top half of the front page of the employee’s most recently filed federal tax return that includes this spouse;
2) NJ Civil Union Certificate or NJ Certificate of Domestic Partnership AND a copy of a recent bank statement or bill that includes both partner’s names received at the same address;
3) Birth Certificate for each dependent natural child showing employee’s name;
4) Birth Certificate for each dependent step child AND a copy of the marriage certificate/partnership certificate showing the employee & parent’s name;
5) Affidavits of Dependency , Final Court Order with presiding judge’s signature and seal OR Adoption Final Decree with presiding judge’s signature and seal for each Legal Guardian, Adoption, Grandchild, or Foster Child;
6) Social Security Disability award AND Federal Tax Return for year just filed for each dependent child with a disability over the age of 23;
7) AND, for Continued Coverage for Over Age Children, that child’s Federal tax Return for year just filed AND, if the child lives outside the State of NJ, documentation of full-time student status must be submitted.
