    • Enhanced Avenue only: $25 application fee (made out to CCM)
  2. To be given to high school for completion and submission:
    • DOCUMENTATION REQUEST:
      Give green sheet to HS Child Study Team or guidance counselor

    • TRANSITION CHECKLIST:
      Give yellow sheet to someone at school who knows you well: HS Guidance counselor, member of Child Study Team, or a teacher

    • OFFICIAL HIGH SCHOOL TRANSCRIPT (no Horizons form):
      Request for transcript usually made through HS Guidance office (should include HSPT scores and SAT scores if available)
