The following is a list of forms and processes administered through the Office of Records & Registration.  Those forms in bold are available in a PDF printable version.

Please print all pages for each form.  Instructions will be included on the form or on a supplemental page.  Please refer to the College Catalog or the Academic Policies site for additional information.

• Change of Major  * - Policy
• Change of Information
  • Name, address, phone number
• Drop/Add Classes  -  Policy  -  Refund Schedule
• Enrollment Certification Form
• Request for Academic Bankruptcy - Policy
  • For students who have not attended CCM for at least 5 years.
• Request to Drop Grades due to Change of Major - Policy
