COVID Exemption Forms

All employees must either:

1. Submit your COVID-19 vaccine record to hrcovid@ccm.edu or
2. Request an exemption form agreeing to take a COVID-19 PCR test weekly and present results weekly to hrcovid@ccm.edu

To request an exemption form, please select one of the following forms.  Completed forms should be sent directly to HRCOVID@ccm.edu.  Responses to requests are usually returned with 48 hours.

 

• Personal Belief Exemption
• Religious Exemption
• Medical Exemption
