For students who need to update their personal information, have attended a New Jersey high school, have moved into Morris County or are without lawful immigration status need.
Students may request changes to their information by using these forms:
Change of Personal Information Form should be used when changing the following:
- Name (documentation needed – social security card or marriage license or court documents)
- Address
- Phone
- Social Security Number (documentation needed – social security card)
Chosen Name Form should be used when students want to use a Chosen Name rather than their legal name. Visit our Student Records page for information about Chosen Name.
Morris County Residency Declaration Form should be used when a student has moved to Morris County and is declaring Morris County residency. See form for documents that are needed to prove Morris County residency.
Request for In County Tuition Status Form should be used by students without lawful immigration status.
