Application Steps
  1. Complete the regular CCM Admissions Application:  Apply online and wait to receive notification directly from Admissions on your acceptance..
  2. Complete the Accessibility Services Application. You cannot complete this online.  Please click here to print a copy to complete.
  3. Include all needed documentation with your Accessibility Services Application. For a list of applicable documentation please click here.
  4. You can email, fax or drop off your documentation to the office or mail your completed application along with needed documentation* to:

Once your file is complete, you will receive a phone call from the Accessibility Services Office regarding your placement test. You will be asked to meet with an Accessibility Support Specialist for the interactive intake process to review accommodations. After this meeting, you can schedule your placement exam with accommodations.

PLEASE NOTE: The Admission Office will send you a letter/email regarding the Placement Test. Please wait until you meet with Accessibility Services Office to schedule your test. You must take your Placement Test with the Accessibility Services Office in order to receive accommodations.
