Students who wish to utilize their accommodations in their classes must submit the Accommodation Letter Request Form each semester. This will grant permission for the office to create your accommodation letters. Please register with the office by submitting an Accessibility Services application and the appropriate documentation before requesting accommodation letters. If you have already completed this process, you do not need to apply again for services. Please fill out the form and you may either submit it by e-mail to firstname.lastname@example.org or submit it in person to the Accessibility Services Office in LRC 105.
